Patient Bill of Rights

The Patient Bill of Rights was developed specifically for individuals who use the services of the Energy Square Prosthodontics

1. You have the right to a screening review of your medical, dental & personal history as may impact your dental treatment.
2. You have the right to a complete oral exam of all hard & soft tissues.
3. You have the right to a series of diagnostic radiographs as indicated.
4. You have the right to a screening exam for head and neck cancer.
5. You have the right to a screening exam related to general health & its potential impact on dental treatment.
6. You have the right to be informed of any condition noted that may affect your dental treatment.
7. You have the right to be informed of potential dental treatment options with associated potential advantages, disadvantages, risks and complications.
8. You have the right to expect that the dental treatment is intended to maintain and/or improve your oral health.
9. You have the right to be informed of dental treatment prognosis.
10. You have the right to be informed of dental treatment costs.
11. You have the right & obligation to expect to sign a consent form prior to initiating dental treatment.
12. You have the right to have the dental treatment completed in a safe environment.
13. You have the right to expect the dental treatment to be delivered in a reasonable timely & competent fashion.
14. You have the right to appropriate & timely referrals.
15. You have the right to confidentiality & control over the release of your patient information.
